Findings: Alleged Perpetrator 2 (AP2) failed to provide a safe medication administration system when s/he left approximately 16 pills unsupervised with residents. The failure resulted in the Alleged Victim (AV) ingesting the medication, experiencing unreasonable discomfort and was transferred to the hospital for treatment. AP2's actions are considered neglect of care and constitutes abuse. The facility failed to assure the safety of residents which is a violation of Oregon Administrative Rules.
