Findings: The facility failed to ensure supervision and staff support regarding known behaviors related to Witness #1 (W1). An incident occurred between the Alleged Victim (AV) and W1 where W1 forcefully pushed open AV's door and hit him/her, causing him/her to fall from his/her wheelchair. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
