Findings: The facility failed to ensure supervision and staff support regarding known behaviors related to Witness #1 (W1). An incident occurred between the Alleged Victim (AV) and W1 where W1 entered AV's room and attacked him/her, causing him/her, hitting and biting, causing bleeding and some injuries. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
