Findings: Witness 1 (W1) had known aggressive behaviors and the facility failed to care plan according to those behaviors. On or about October 4, 2019, W1 made a fist and struck Alleged Victim (AV) in the mouth resulting in AV's getting a bloody, swollen lip and a loose front tooth. The facility failed to put sufficient interventions in place to protect AV from W1's inappropriate aggressive behavior which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP20-00647 $375.00 fine assessed
