Findings: The facility failed to implement appropriate interventions and staff support regarding known fall risks related to the Alleged Victim (AV). AV has had multiple falls resulting in bruises, some which may have been due to a medication change for AV, however, no new interventions were put in place to staff to know how to reduce the risk of falls for this resident. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
