Findings: Alleged Victim (AV) had been agitated all morning, has a history of resident to resident altercations and was receiving one on one care with staff. Staff had to leave for a meeting. AV then engaged in a resident to resident altercation. Staff did not take AV to a quiet place or engage in an activity as directed in the care plan. The facility failed to follow AV's careplan, which is a violation of resident rights, is neglect of care and constitutes abuse.
