Findings: The facility failed to implement interventions and appropriately care plan related to the Alleged Victim's (AV) known fall history. On or about October 6, 2019, AV experienced multiple falls to which s/he was transported to the hospital and diagnosed with a fractured hip. The failures are a violation of resident rights, is considered neglect of care and constitutes abuse.
