Findings: Alleged Perpetrator 2’s (AP2) job duty was to administer and document Alleged Victim’s (AV) ordered medication. On several occasions in October 2019, AP2 failed to properly administer two of AV’s ordered medication. There is no facility documentation indicating that AP2 completed medication administration training. AP2's actions are considered neglect of care and constitutes abuse. The facility’s failure to provide a safe medication administration system placed AV at risk of harm, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
