Findings: The facility failed to address Alleged Victim's (AV) aggressive behaviors. From July 24, 2019 through October 8, 2019, AV was involved in fourteen (14) resident-on-resident incidents. On or about October 10, 2019, AV and Witness 5 (W5) had altercation in which AV incurred a laceration to his/her scalp. The facility failed to put sufficient interventions in place for AV to protect others from AV's aggressive behavior, which ultimately led to the October 10, 2019 incident. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
