Findings: On or about October 13, 2019, care staff discovered bruising on Alleged Victim's (AV's) arms. AV reported he/she had been "man handled" during the night. Alleged Perpetrator 2 (AP2) was witness to be struggling with AV by another staff. AP2. AP2 reported that AV was being combative with cares. AP2, instead of re-approaching continued to provide care while AV was being aggressive and yelling for someone to "get away from me." AP2's actions are considered neglect of care and constitutes abuse. AP2 had a known history of being rough or frustrated with residents. The facility failed to provide a safe environment, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
