Findings: On or about October 25, 2019, Alleged Perpetrator #2 (AP2) was transferring the Alleged Victim (AV) in the bathroom. The gait belt was too loose, when AV tried to get up by him/herself, AP2 could not control the fall due to the gait belt being on too loose. AV fell causing multiple injuries, including a broken rib. AP2's actions are considered neglect of care and constitute abuse. The facility failed to provide a safe environment for the AV by not properly training AP2 prior to having this staff use a gait belt with residents. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: ALFCP20-00250 $375.00 fine assessed
