Findings: The facility failed to ensure the Alleged Victim's (AV) care needs were being met due to a lack of training for catheter care. Alleged Perpetrator #2 (AP2) did not receive proper training on catheter care and while performing care for AV, AP2 caused unnecessary irritation to AV by improperly caring for AV. The facility's failure to provide proper training is a violation of resident rights, is considered neglect of care and constitutes abuse.
