Findings: On or about November 5, 2019, Alleged Victim (AV) eloped from the facility. AV was found and returned by police and AV suffered injuries to his/her face and hand. An investigation determined that the facility had problems with the west side door latching properly and the facility staff had known of the issues prior to AV's elopement. On the day of the incident, Alleged Perpetrator 2 (AP2) left the facility knowing the west side door did not latch properly and did not ensure the west side door was secure when leaving the building which lead to AV eloping from the facility. AP2's actions are considered neglect of care which constitutes abuse. The facility failed to provide AV a safe environment which is a violation of resident rights, is considered neglect of care and constitutes abuse.
