Findings: On or about November 3, 2019, Alleged Perpetrator #2 (AP2), Alleged Perpetrator #3 (AP3) and Alleged Perpetrator #4 changed the Alleged Victim's (AV) clothing and incontinence undergarments in the common area of the facility, while other residents were present. The AV suffered a loss of dignity due to this action. AP2, AP3 and AP4's actions are a violation of resident rights, considered neglect of care and constitute abuse. The facility's failure to protect AV from a loss of dignity is a violation of resident rights, is considered neglect of care and constitutes abuse.
