Findings: Alleged Victim (AV) has a known history of wandering into other resident's rooms, resulting in resident to resident altercations and is care planned for staff to redirect his/her away from others rooms when wandering. On or about November 11, 2019, AV wandered into Witness 1's (W1's) room. W1 aggressively pushed AV out of his/her room. The facility failed to follow AV's care plan which put AV at risk for serious harm, which is a violation of resident rights, is neglect of care and constitutes abuse.
