Findings: Witness 1 (w1) has a known history of wandering into other residents room. Staff are to redirect W1 when he/she is wandering and immediately intervene. There are signs to keep W1 out of some residents rooms, but W1 does not pay attention to them. W1 has a known history of resident to resident altercations. On or about November 12, 2019, W1 was attempting to enter an apartment that was not his/her when AV approached W1 and said "that is not your room." W1 turned around and pushed AV with his/her walker and then pushed AV again with his/her hands. AV fell to the floor and landed on his./her hip. On or about November 16, 2020, W1 and Alleged Victim (AV) got into an altercation again after W1 entered into AV's room, taking AV's crackers. The facility failed to provide a safe environment, which is a violation of resident rights, is neglect of care and constitutes abuse.
