Findings: Alleged Victim (AV) and Witness 1 (W1) have a known history of resident to resident altercations. AV is care planned to be redirected from going into other residents' room. On or about November 13, 2019, staff responded to screaming heard in W1's room and found W1 and AV had had an altercation hitting each other. The facility failed to follow the care plan, which is a violation of resident rights, is neglect of care and constitutes abuse.
