Findings: On or about November 15, 2019, Alleged Perpetrator #2 (AP2), Alleged Perpetrator #3 (AP3), and Alleged Perpetrator #4 were trying to clean up the Alleged Victim (AV) after an instance of incontinence. During the action, AV became combative with staff. Instead of leaving and re-approaching AV as outlined in his/her care plan, AP2, AP3 and AP4 determined to force AV to be cleaned up against his/her will, causing added stress and bruising to AV's wrists and arms. AP2, AP3 and AP4's actions are considered neglect of care and constitute physical abuse. The facility failed to provide a safe environment, which is a violation of Oregon Administrative Rules.
