Findings: On or about January 21, 2020, Alleged Victim (AV) left the facility through a gate that was left unsecured by a construction worker. Witness 2 (W2) immediately saw AV walking down the sidewalk, notified staff and stayed with AV until staff arrived. AV was brought back into the facility unharmed. The facility failed to provide a safe environment which put AV at risk of serious harm, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: RCFCP20-00644 $188.00 fine assessed
