Findings: Witness 1 (W1) has a known history of behaviors. On or about November 14, 2019, W1 cussed at and pushed Alleged Victim (AV) while on a facility bus getting ready for an outing. W1's care plan states anger/aggression towards other residents and states W1 is to be at a table by W1's self or with significant other due to these behaviors. Given this information, W1 having a behavior is a confined environment such as a facility bus was predictable and preventable. AV has altered his/her activities due to fear of running into W1. The facility failed to provide a safe environment, which is a violation of resident rights, is neglect of care and constitutes abuse.
