Findings: The facility failed to provide a safe medication administration system to ensure the Alleged Victim received medication as ordered. On or about November 23, 2019, Witness #1 gave the incorrect type of insulin and the incorrect dosage of the medication to the Alleged Victim, placing him/her at risk for harm. The Allege Victim was sent out to the hospital for observation. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
