Findings: Alleged Victim (AV) was care planned to wear hip protectors due to fall risk. On or about November 23, 2019, AV fell resulting in a hip fracture. AV's hip protectors were not ordered or placed on AV as an intervention. The facility failed to follow AV's care plan which is a violation of resident rights, is considered neglect of care and constitutes abuse.
