- Findings
- Alleged Victim (AV) was a known fall risk. From approximately July 21, 2019 through November 23, 2019, AV experienced approximately 32 falls, several with injuries and trips to the emergency room. The facility failed to care plan and implement effective interventions to mitigate AV's risk of falls causing AV repeated unreasonable discomfort, which is a violation of resident rights, is neglect of care and constitutes abuse.
