Findings: Alleged Victim (AV) has a known history of intervening in resident to resident altercations and cleaning up after meals or activities. On or about November 27, 2019, AV attempted to take a cup from where Witness 1 (W1) was sitting and W1 scratched AV causing bruising and a skin tear. No facility staff witnessed the event. The facility failed to provide a safe environment, which is a violation of resident rights, is neglect of care and constitutes abuse.
