Findings: The facility failed to provide proper supervision to maintain the health and safety of Alleged Victim (AV) which resulted in AV falling when he/she attempted to transfer unassisted. An investigation determined that AV's care plan was not being followed at time of incident which is a violation of resident rights, is considered neglect of care and constitutes abuse.
