Findings: Witness 1 (W1) and Alleged Victim (AV) were known to have altercations and were care planned to be monitored and kept separated. On or about December 7, 2019, W1 and AV had an altercation in which W1 attempted to harm AV by placing his/her hand on AV's throat. An investigation determined that prior to incident, W1 was expressing intention to cause serious physical harm towards AV and the facility failed to care plan for AV's safety prior to the altercation. The facility failed to put sufficient interventions in place which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: ALFCP20-00283 $375.00 fine assessed
