Findings: Witness 1 (W1) and Alleged Victim (AV) were known to have altercations and were care planned to be monitored and kept separated. On or about December 7, 2019, W1 and AV had an altercation in which AV attempted to strangle W1. An investigation determined that prior to incident, AV was expressing homicidal ideations towards W1 and the facility failed to care plan for W1's safety prior to the altercation. The facility failed to put sufficient interventions in place which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: ALFCP20-00282 $375.00 fine assessed
