- Findings
- Alleged Victim (AV) service plan described AV as able to ambulate without assistive devices and independent with transfers. AV experienced non injury falls on or about October 10, 2019 and December 6, 2019. AV experienced falls resulting in injury on or about September 11, 2019, November 26, 2019, December 6, 2019 and twice on December 7, 2019. Temporary service plans observed in the 24 hour communication log addressed medical care of AV's injuries. The facility failed to care plan and implement interventions to mitigate AV's risk of injury due to falls, which is a violation of resident rights, is neglect of care and constitutes abuse.
