Findings: The facility failed to follow Alleged Victim's (AV) care plan which resulted in the facility not following through with AV's diet order. The facility failed to provide reminders/cueing for nutrition/hydration which caused AV to become dehydrated and suffer from acute kidney failure. The facility's failure to follow AV's care plan resulted in AV being hospitalized for treatment. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
