Findings: The facility failed to provide a safe environment by not having adequate supervision according to Alleged Victim’s (AV) known behaviors and service plan. On or about December 8, 2019, AV and Witness 1 (W1) were involved in an injury altercation when W1 became upset with AV and hit him/her, causing AV to fall and sustain a wrist fracture. The failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP20-00360 $188.00 fine assessed
