Findings: On or about December 3, 2019, Alleged Perpetrator #2 (AP2) failed to follow the Alleged Victim's (AV) care plan to transfer him/her using two people AP2 tried to contact other staff to help him/her with the transfer, however, there were not enough staff available to help. AP2 determined to try to transfer AV by him/her self and during AP2's transfer, AV lost his/her footing and AP2 completed a controlled fall. Although AV was not harmed, he/she was placed at a risk for serious harm. AP2's actions are considered neglect of care and constitute abuse. The facility failed to have enough staff available to meet the needs of AV, putting AV and other residents at a risk for serious harm. The facility failures are a violation of resident rights, are considered neglect of care and constitute abuse.
