Findings: Alleged Victim (AV) was a known fall risk. On or about November 28, 2019, AV fell and used his/her call button to request assistance. Alleged Perpetrator 2 (AP2) responded but wouldn't help AV. AV reported this made him/her to feel scared and feel a loss of dignity. AP2's actions are considered neglect of care and constitutes abuse. The facility was aware of multiple issues with AP2 prior to this incident including but not limited to errors in medication administration, suspicion of involvement of missing medications, not following facility policy and not performing AP2's duties while working. The facility failed to act timely in addressing AP2's employment at the facility put AV at risk of serious harm, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: ALFCP20-00423 $500.00 fine assessed
