Findings: On or about October 7, 2019, Alleged Victim (AV) received a new doctor's order for his/her blood thinning medication. Due to staff error the order was not initiated until approximately December 12, 2019, putting AV at risk for serious harm. The facility failed to administer a medication as ordered, which is a violation of resident rights, is neglect of care and constitutes abuse.
