Findings: Alleged Victim (AV) has a known history of falls, skin tears, and fragile skin. AV experienced falls on or about November 13, 2019, November 27, 2019, November 28, 2019, November 30, 2019 and December6, 2019. During that period of time the only new intervention put into place was to place AV in his/her bed or recliner if he/she was sleeping in his/her wheelchair. The November 28, 2019, fall was from AV sleeping in his/her wheelchair. On or about December 12, 2019, AV was found to have scattered bruising from a suspected fall. The facility failed to care plan and implement appropriate interventions to mitigate AV's risk of injury due to falls which is a violation of resident rights, is neglect of care and constitutes abuse.
