Findings: Alleged Victim (AV) was a fall risk and had a history of falls. On or about December 16, 2019, AV had an unwitnessed fall, was transported to the hospital and diagnosed with two different brain bleeds. AV also suffered skin tears and bruising. The facility failed to care plan and implement appropriate interventions to mitigate AV's risk of injury due to falls, which is a violation of resident rights, is neglect of care and constitutes abuse.
