Findings: On or about December 12, 2019, the Alleged Victim (AV) sustained a un-witnessed fall, which resulted in a cut and two abrasions on his/her nose. The facility's care plan did not provide for interventions regarding AV's self-transferring or refusal to wear his/her oxygen. AV's physician noted that AV was having cognitive decline and difficulty with memory as early as July 2019, however, AV's care plan was not updated with additional information to ensure AV was being cared for properly. The facility's failure to properly care plan for AV is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: ALFCP20-00363 $450.00 fine assessed
