- Findings
- The facility failed to properly plan care for Alleged Victim's (AV) risk of falls. An investigation determined that AV suffered from three to four falls between December 17, 2019 and December 19, 2019. The facility failed to appropriately care plan and implement interventions for AV to reduce the risk of falls. The facility's failure to properly plan care for AV is a violation of resident's rights, is considered neglect of care and constitutes abuse.
