Findings: On or about December 16, 2019, Alleged Victim (AV) and Witness 1 (W1) were involved in an altercation, resulting in W1 grabbing and twisting AV’s wrist, causing AV to call out for help. W1 had a history of aggressive behaviors towards other residents and his/her care plan did not have sufficient instructions on how to monitor or prevent W1 from these behaviors. AV had known behaviors regarding W1 and his/her care plan did not have sufficient instructions on how to monitor or redirect AV from these behaviors. The facility's failure to provide adequate supervision and implement appropriate interventions are a violation of resident rights, are considered neglect of care and constitutes abuse.
