- The facility failed to provide appropriate supervision of the Alleged Victim (AV) according to his/her known behavior and prior altercation. The facility resulted in another physical altercation with AV causing unreasonable discomfort, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
