Findings: Alleged Victim (AV) is care planned for shower assistance. On or about December 28, 2019, approximately 7:40am staff heard that AV was in the shower but staff did not talk to AV. The next check was completed at approximately 8:15am at which time staff verbally checked in with AV, who was still in the shower, and AV verbally responded to staff. At approximately 9:00am AV was found in his/her shower unresponsive. AV developed blisters/burns from the hot water running on his/her legs and was transferred to the hospital for treatment. The facility reported that AV was independent with showers and was not on the shower schedule, however AV had a similar incident July 18, 2018. AV's care plan indicates AV requires stand by assistance with showers effective August 1, 2017. The facility failed to follow AV's care plan, which is a violation of resident rights, is neglect of care and constitutes abuse.
