Findings: Alleged Victim (AV) resides in a secured facility and is known to have exiting seeking behavior. On or about December 20, 2019, AV exited the building unassisted. The facility determined the mechanical issues with the door. On or about December 31, 2019, AV exited the building through the same door and was gone for an undetermined amount of time. Approximately seven days later the investigator observed the door was still not functioning properly and the facility still allowed visitors and staff to use the door. The facility failed to provide a safe environment putting AV at risk for serious harm, which is a violation of resident rights, is neglect of care and constitutes abuse.
