- Findings
- On or about January 1, 2020, Alleged Perpetrator #2 (AP2) failed to follow the Alleged Victim's (AV) care plan while he/she was assisting the AV with getting dressed and transferring out of bed. AP2 left the AV on the edge of his/her bed to get another caregiver when AV fell and sustained a fall with injury. AP2's actions are considered neglect of care and constitute abuse. The facility's failure is a violation of Oregon Administrative Rules.
