Findings: Alleged Perpetrator 2 (AP2) grabbed a hold of the Alleged Victim (AV) by his/her arm, in an attempt to keep him/her from leaving the memory care unit, causing undue mental distress and a red mark to AV's arm. AP2 is responsible for physical abuse, which is a violation of residents rights, is considered neglect of care and constitutes abuse. The facility failed to protect AV from physical abuse, and the facility's failure is a violation or Oregon Administrative Rules. The investigation inconclusively determined that the facility did not have a responsibility of abuse.
