Findings: The facility failed to ensure supervision and implement meaningful and effective interventions in place regarding Witness #1 (W1) and his/her many altercations with residents. On or about January 3, 2020, W1 was in the dining area with the Alleged Victim (AV) when W1 grabbed and twisted AV's wrist causing pain to AV. The facility's failure to provide a safe environment is a violation of resident rights, is considered neglect of care and constitutes abuse.
