Findings: In December 2019, Alleged Victim (AV) experienced a change of condition and remained in bed due to pain with any movement as the facility made ongoing requests to AV’s doctor to manage his/her pain. AV was care planned to be repositioned and to have a pillow positioned under his/her feet to avoid all contact between his/her heels and the bed, to prevent pressure ulcers. The facility failed to provide adequate repositioning and failed to appropriately position AV’s feet/heels, resulting in AV developing pressure ulcers on both heels and redness around his/her coccyx. The facility’s failure to follow AV’s care plan is a violation of resident rights, is considered neglect of care and constitutes abuse.
