Findings: Alleged Perpetrator #2 (AP2) was trying to assist with giving the Alleged Victim (AV) a shower, pulled a band roughly from AV's wrist, causing injury and forcibly pushed AV into the shower. AP2's actions are considered a violation of resident rights, is considered neglect of care and constitutes physical abuse. The facility failed to provide a care plan for staff to refer to in order to know how to handle the AV during showering and failed to protect the AV from rough treatment, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
