Findings: The facility failed to provide a safe environment for Alleged Victim (AV) which resulted in Alleged Victim (AV) sustaining scratches, a bloody nose, and red marks. On or about January 5, 2020, Witness 1 (W1) entered AV's room and aggressively harmed AV. An investigation determined that W1 had an altercation with another resident earlier in the day of the incident and was redirected away from that resident to ensure no new altercations took place. However, the investigation determined that W1 was not checked every 15 minutes as care planned. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
