Findings: The Alleged Perpetrator 2 (AP2) used wrongful restraints against the Alleged Victim (AV) due to his/her known fall history. The failure resulted in AV experiencing unreasonable discomfort and a loss of personal dignity. AP2 is responsible for wrongful restraint which constitutes abuse. The facility failed to protect AV which is a violation of Oregon Administrative Rules.
