Findings: The facility failed to appropriately care plan for the Alleged Victim (AV) regarding his/her risk of falls. AV was found on the floor multiple times from December 30, 2019 to January 6, 2019. No meaningful interventions were put into place to keep AV from falling until he/she was seen by his/her PCP. On January 6th, 2020, AV was transported to the hospital for treatment and was diagnosed with sprained wrist. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
