Findings: On or about December 23, 2019, Alleged Perpetrator 2 (AP2) attempted to take an alcoholic beverage from the refrigerator of the Alleged Victim (AV). AV confronted AP2, at which time, AP2 then physically grabbed and shoved AV up against the wall in an attempt to keep AV from telling others what he/she found AP2 doing in his/her apartment. AP2 is responsible for physical abuse, which is a violation of resident rights, is considered neglect of care and constitutes abuse. The facility failed to protect AV#1 from physical abuse by staff, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
